Prepare for your assessment

  • Ensure that you – and/or others in your practice – are ready to answer the key risk management questions outlined in our assessment guidelines.
  • Prepare items – which might include log books, policies and procedures, job descriptions and other documents – that you will share with us to show you are complying with our guiding principles.
  • Prepare medical record documentation –  Please pull or identify one medical record for each physician or clinician with your practice.
    • For all specialties except radiology, pathology and anesthesia, records pulled or identified should be of patients who require reasonably complicated care, for example patients with multiple comorbidities, who have been seen several times over the last year.
    • For radiologists, records pulled or identified should be for a patient with a critical or significant result.
    • For pathologists, records pulled or identified should be for a patient with a significant or unexpected finding.
    • For anesthesiologists or CRNAs, records pulled or identified should be for a patient undergoing an invasive procedure who has the highest ASA physical status classification you treat.

 

Note, if you are using an electronic medical record system, you need not print the records. Rather, we will need a list of the records you have identified and access to your electronic medical record system.
